The Role:

As a Clinical Team Leader you will perform an integral role under Support At Home and the Aged Care sector. You will lead a small team to ensure individualised services are developed and delivered to participants based on their assessed clinical care needs. The ability to build rapport with the participant and their families advocates and health professionals in this process is essential.

You will be required to

Manage and motivate a small team of Care Partners in the delivery of exceptional customer service.
Complete comprehensive nursing assessments capturing all relevant information.
Develop monitor & review individual and other associated health care plans.
Oversee care and be the main liaison and advocate for Support at Home participants.
Advocate on behalf of the participant.
Provide ongoing Training and Support to staff.
