Job Description
- Collaborate with multidisciplinary clinical teams to enhance patient safety by ensuring safe appropriate and cost-effective use of medications including chemotherapy and immunosuppressive therapies.
- Conduct audits of patient medical and medication records to identify medication errors drug interactions adverse drug events and associated risks; recommend corrective and preventive strategies.
- Perform Drug Utilization Reviews (DUR) to evaluate medication usage patterns and ensure rational prescribing.
- Identify analyze prevent and manage drug-related problems through evidence-based patient-centric medication therapy recommendations.
- Participate in interdisciplinary patient care rounds including transplant units and critical care areas and communicate therapeutic interventions with the clinical team.
- Assist the HOD and hospital teams in developing and implementing medication management policies and procedures.
- Address medication-use process concerns through active participation in relevant hospital committees.
- Coordinate and participate in monthly Pharmacy & Therapeutics Committee meetings.
- Participate in interdepartmental meetings and fulfill assigned responsibilities.
- Manage and support the Antibiotic Stewardship Program.
- Act as Chapter Champion JCI Medication Management and Use (MMU).
- Ensure implementation and compliance with all quality and accreditation standards related to medication management.
- Prepare and maintain departmental MIS as per the defined calendar.
- Perform additional duties as assigned by the HOD from time to time.
