Job Summary

GENERAL DESCRIPTION

This position provides intensive case management to youth to help them achieve their goals while ensuring a safe space while engaging with HYS staff. The case manager helps youth learn life skills build positive relationships and navigate public and social services available in the community such as mental and physical health services employment education and housing services.

MINIMUM QUALIFICATIONS

The requirements listed below are representative of the knowledge skill and/or ability required.

EDUCATION and/or EXPERIENCE

  • High School diploma or General Education Degree (GED).
  • Four or more years of work or volunteering experience in fast-paced customer service healthcare education social or community services or related public-facing sectors.
  • College course work in social sciences or related human services preferred but not required.
  • Preference will be given to candidates with work or volunteer experience working with a special needs population and/or experience working with youth or high-risk youth 11-18 years of age.
  • An equivalent combination of education and experience may be considered.

CERTIFICATES LICENSES REGISTRATIONS

  • Driver qualifications required for this position. Valid driver license insured vehicle available for use on the job and a satisfactory drive record for the past 3 years are required.
  • Candidate must pass Oregon Department of Human Services criminal background check.
  • CPR/First Aid Food Handlers card mandatory reporting privacy laws trauma informed care suicide intervention collaborative problem solving and recognizing and reporting child abuse & neglect training and/or certifications required within the first 30 days.
  • Candidate must pass pre-employment and random drug/alcohol screenings.

KNOWLEDGE SKILLS AND ABILITIES

  • Ability to build and maintain healthy boundaries and relationships with youth and community partners.
  • General understanding of de-escalation role-modeling and active-listening.
  • Proficiencies in the use of Microsoft Office such as Word and Outlook.
  • Effectively communicates orally and in written form; read and interpret policies and instructions.
  • Ability to utilize video conferencing navigating and communicating on social media and general technology skills.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assists youth in developing and achieving their goals with a focus on health education employment and housing.
  • Conducts assessments with youth to determine strengths needs service and housing eligibility etc.
  • Advocates for and obtains services for youth and their families.
  • Maintains a necessary level of involvement in youth activities such as driving youth to appointments sitting in on wrap-around service youth meetings and/or sitting in on meetings at school to help ensure youth are progressing in obtaining services and working toward their goals.
  • Provides necessary transportation to youth using program vehicle.
  • Organizes and supports youth activities and workshops in alignment with HYS program needs.
  • Provides educational support to youth in alignment with curriculum development.
  • Oversees youth internships in accordance with program policies and procedures.
  • Provides resource navigation for clients and youth in the community ensuring access to housing assessments and resources.
  • Maintains and updates all necessary databases for tracking youth progress program objectives and outcomes and contractual and licensing requirements.
  • Answers the phone: directs calls or takes messages appropriately (land line cell phone and social media).
  • Effectively presents information and responds to questions from clients customers partners and the general public regarding program services.
  • Completes timely data entry for intake call logs client notes and exit paperwork in an accurate and professional manner.
  • Accurately documents and reports all services both in print and electronically in adherence to agency and licensing requirements.
  • Collects and responds to youth run reports filed with law enforcement.
  • Extends services to youth after they exit the program.
  • Conducts street outreach with HYS staff and community partners. Represents HYS at community outreach events.
  • Maintains building appearance by completing necessary tasks for site upkeep before shift change.
  • Applies creativity and problem solving in daily activities and dealing with challenges.
  • Complies with MWVCAA safety and personnel policies including punctuality and attendance.

PHYSICAL AND MENTAL DEMANDS

  • Specific vision abilities required by this job include close vision and ability to adjust focus.
  • Frequently lift up to 25 pounds. Occasionally lift up to 50 pounds.
  • Manual dexterity for handling computers devices and office equipment.
  • Maintains calm dispositions and regulates emotions when clients staff or others may become escalated.
  • Regulates emotions and handles pressure of frequent demands for attention time and work tasks.
  • Personally responsive to urgent situations which could require travel for on-site interventions.
  • Ability to exercise judgement and quick decision-making skills in response to guest or client incidents.

WORK ENVIRONMENT

  • Close quarters often with a population experiencing homelessness with additional barriers such as mental illness and substance use.
  • Frequent noise and distractions in workspaces.
  • Indoor and outdoor work environment.
  • May work in kitchen laundry and restroom spaces utilizing basic tools and equipment for necessary site upkeep.
  • Ability to work flexible hours including nights weekends and holidays as needed and scheduled by supervisor.
