Provider must have 3 years minimum of current Pharmacy Technician experience - REQUIRED
AZ State Board of Pharmacy License and National Pharmacy Technician Certification - REQUIRED
Certifications must be American Heart Association or American Red Cross - REQUIRED
License cannot show any hits on license at time of submission - REQUIRED DOCUMENT
Banner will not approve RTO within the first week or holidays. Can not exceed 7 days - REQUIRED
