About the role:
The Hospital Social Worker offers high level assistance to patients and their families with practical and emotional challenges relating to their illness hospitalisation and discharge planning. They provide support information and referrals and may offer counselling and mediation services. They will work as part of a multidisciplinary team to ensure our patients needs are met.
