Your Opportunity
As a Case Manager youll be part of an exciting new Mission Australia program based on-site at The Grant High School offering a unique opportunity to help shape and deliver a newly established school-based initiative supporting young people with complex needs. You will provide hands-on strengths-based case management managing a caseload of around 2025 students and working directly with approximately five students each day to set meaningful term-based goals support re-engagement in education and explore alternative education training or employment pathways. Working closely with school wellbeing teams and families youll use your initiative sound judgment and genuine passion for youth outcomes to help students build positive futures while maintaining accurate records and adhering to privacy and confidentiality requirements. This is a maximum-term full-time contract until 31 December 2026 with the potential for extension subject to program success and outcomes achieved.
Your key responsibilities will be to:
Assist clients through direct support interventions and referrals including helping them transition towards independence or other programs.
Deliver person-centred case management to young people with complex needs supporting them to set and achieve individual goals and reviewing progress against case plans.
Assist clients to re-engage with education training or employment pathways and provide referrals to appropriate support services.
Develop holistic strengths-based individualised support plans in collaboration with students families and school teams.
Maintain accurate client records and administrative documentation ensuring compliance with privacy confidentiality and program requirements.
Build and maintain positive relationships with students families school staff and external services contributing to a collaborative supportive environment.
Requirements for Success
A minimum qualification of Certificate IV or higher in youth work case management or a related field.
Ability to provide specialised or complex therapeutic interventions to young people.
Experience in outreach mentoring and supporting youth to develop independence.
Proven ability to build positive relationships with students families school staff and external services.
Strong communication organisational and administrative skills.
Substantial experience in case management youth work or a related field ideally in school or education settings.
Hold a Current SA full Drivers Licence and willingness to obtain a First Aid Certificate.
Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
