Qualifications / Requirements:

MD/DO
Unrestricted State Medical License
Unrestricted DEA License or ability to obtain
OB/GYN BC/BE (ABOG AOBOG)
Board certification required within 5 years of eligibility
BLS

Active and Unrestricted Florida License (or ability to obtain a Florida License)
Completion of an ACGME Accredited Gynecologic Oncology Fellowship
Completion of an ACGME Accredited Obstetrics and Gynecology Residency
