ilumed에서는 원격 돌봄 관리 팀에 합류할한국어와 영어를 구사하는 등록 간호사를 구합니다. 해당 직무에서는 Medicare 수혜자에게 전화 돌봄 조정 질병 관리 지원 환자 교육을 제공하여 임상 결과를 개선하는 동시에 수혜자가 자신감을 가지고 건강을 관리할 수 있도록 돕습니다.

직무

• 수혜자와 간병인에게 전화로 돌봄 안내 교육 및 지원 제공
• 의사 및 돌봄 팀과 협력하여 통합 돌봄 계획을 수립
• 동기 부여 인터뷰를 통한 환자 참여 유도 및 돌봄 요구 사항 파악
• 통화량 사례 부하 및 문서화에 대한 돌봄 관리 KPI 충족
• SDOH 요구 사항 파악 및 환자와 적절한 자원과의 연결 구축
• HIPAA 및 규제 표준 준수 보장

자격 요건

• 한국어와 영어에 능통(말하기와 쓰기 모두)
• 해당 직무에는컴팩트 RN 면허뿐만 아니라 캘리포니아주와 뉴욕주 RN 면허도 필요합니다..
• 아직 이 중 하나 이상이 없더라도 괜찮습니다. 적합한 지원자에게는 면허 취득 비용을 기꺼이 부담해 드리겠습니다.

추가 요건:

• 24년의 사례 관리 가정 건강 관리 또는 이와 유사한 RN 경험
• Medicare 또는 Medicare Advantage 유경험자 우대
• 강력한 비판적 사고 의사소통 및 시간 관리 기술
• Microsoft Office 활용 능력 및 EMR에 대한 이해

Bilingual Korean/English-speaking Registered Nurse (Telephonic RN Care Coordinator)

ilumed is seeking a bilingual BilingualKorean/English-speaking Registered Nurse to join our remote Care Management this role you will provide telephonic care coordination disease management support and patient education to Medicare beneficiaries helping them navigate their health with confidence while improving clinical outcomes.

What Youll Do

• Provide telephonic care guidance education and support to beneficiaries and caregivers
• Collaborate with physicians and care teams to develop integrated care plans
• Use motivational interviewing to engage patients and identify care needs
• Meet care management KPIs for call volumes caseload and documentation
• Identify SDOH needs and connect patients with appropriate resources
• Ensure compliance with HIPAA and regulatory standards

What You Bring

• Fluent in Korean & English (spoken and written)
• This role requires a Compact RN License as well as California and New York state RN licenses.
• If you dont have one or more of these yet thats ok were happy to cover the licensing costs for the right candidate.

In addition:

• 24 years of case management home health or similar RN experience
• Experience with Medicare or Medicare Advantage preferred
• Strong critical thinking communication and time-management skills
• Proficiency with Microsoft Office and familiarity with EMRs
