HOURS: Full-Time
SCHEDULE:Monday-Friday 8am to 4pm
PATE OF PAY: $40.00 - $43.00 per hour
Sign on Bonus: $10000
SUPERVISOR: Assistant Program Director/Program Director
Responsibilities and Duties:
Access and provide necessary medical intervention for all individuals including evaluation documentation and follow-up with physician and Program Director.
Oversee administration storage and documentation of all prescribed medications. Ensure adequate supply levels at all times.
Monitoring certification and supervision of all staff dispensing medication. Ensure appropriate AMAP documentation on MAR sheet ongoing.
Complete initial quarterly and semi-annual nursing assessments according to Gan Kavod policy manual and OPWDD regulations.
Act as liaison to Medical Director Day Program Nurse Dietician and all other ancillary services of medical issues incidents and concerns as appropriate.
Provide in-service training to staff on HIV Hep B Infections Control Incident Review and other trainings according to Gan Kavod policy and OPWDD regulations. al issues incidents and concerns as appropriate.
Participate in development review and update of IPP as part of the interdisciplinary team process. Responsible for relaying IPP medical/therapeutic recommendations to physician for his/her review and consideration.
Remove and dispose of discontinued or outdated medication on an as needed basis.
Supervise the training of individuals in the area of self administering medication as feasible.
Monitor review and instruct staff on infection control policies and procedures. Perform quarterly record keeping of resident/employee infections.
Develop review and update medical care plan based on individual need.
Provide emergency nursing care/assessment of information/referral on behalf of individuals during and outside of regularly scheduled work hours when available.
Perform direct physical examination of all body systems for individuals not requiring a medical care plan on a quarterly basis or more frequently depending on needs.
Complete and keep current Client Specific Medication sheet for AMAP use.
Arrange for and assure completion of all laboratory and other medical testing ordered by the attending physician.
Supply medically related documentation when needed to secure court-orders for medical treatment.
Perform any of the residential nursing duties as requested by Program Director or designee.
Act in place of Coordinator in their absence.
Performs duties according to the highest ethical and moral standards and in adherence with the facilitys corporate compliance plan and all applicable governing laws.
Qualifications
Registered Professional Nurse with current New York State license. Minimum three (3) years experience with geriatric or intellectually disabled individuals.
