Key Responsibilities:

Provide management and delivery of patient care plans

Schedule and Shift Details:

Schedule (Including any on call/weekends): Mon-Fri 8 hour days 1 evening of on-call per week (5pm-10pm) and 1 weekend per month (Sat/Sun)
