Responsibilities
Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for patients with chronic and subacute conditions such as coma ventilator dependence or complex illnesses that require 24-hour medical supervision. With a focus on comfort recovery and rehabilitation CSTCC blends the effectiveness of acute treatment with the supportive environment of long-term care helping patients progress toward improved function and quality of life.
- Perform physical therapy examinations evaluations and individualized treatment planning to restore mobility and improve physical function.
- Participate in interdisciplinary case conferences discharge planning and the coordination of care across the rehab team.
- Implement a plan of care for patients and evaluate the effectiveness of it in an ongoing systematic process.
- Supervise physical therapist assistants rehab therapy aides and student clinical experiences to ensure quality care and professional development.
- Guide therapy and intervention through evidence-based research

Qualifications
Education & Experience
- Bachelors Degree in Physical Therapy required
Licenses and Certifications
- PT - State of California Physical Therapist License
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)
