As a Home Health RN Case Manager heres a big-picture view of what youll do:

• Complete accurate and timely OASIS Start of Care (SOC) Resumption of Care (ROC) Recertification and Discharge assessments

• Develop implement and update individualized patient care plans

• Perform skilled nursing visits including assessments medication management wound care and patient education

• Coordinate care with physicians therapists and interdisciplinary team members

• Ensure compliance with Medicare state and agency regulations

• Maintain thorough and timely clinical documentation in the EMR

• Provide education and support to patients and caregivers

A few must-haves for Home Health RN Case Managers:

• Graduate of an accredited nursing program and active RN license in Kansas
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty strong communication skills and compassion
