Important: This is a Pennsylvania-based remote position. Applicants must reside in PA or hold an active Pennsylvania license. Out-of-state applicants will not be considered.
Job Summary:
The Licensed Clinical Social Worker (LCSW) provides trauma-informed psychosocial assessments care planning and referral coordination for individuals seeking access to mental health services. This role focuses on engagement stabilization and linkage to appropriate Health Choices-funded services without duplicating reimbursable treatment.
Responsibilities:
- Conduct individual and family mental health assessments
- Develop individualized trauma-informed care plans
- Provide short-term supportive counselling and engagement
- Coordinate referrals to mental health housing and crisis services
- Work with hospitals schools juvenile justice and child welfare
- Maintain documentation in CIPS
- Advocate for client rights equity and language access
Requirements:
- Masters degree in Social Work
- Active Pennsylvania LCSW
- 3 years of behavioral health experience
