What Youll Do:
Deliver skilled compassionate in-home therapy as part of a collaborative care team
Evaluate and develop individualized treatment plans for adult and geriatric patients
Serve as a key liaison between patients families and interdisciplinary team members
Document care clearly and thoroughly in our EMR system
Help shape the future of in-home rehabilitation
What You Bring:
Doctor of Physical Therapy (DPT) or equivalent (MPT MScPT)
Current and unrestricted Virginia Physical Therapy license
Two or more years of experience in home health acute care or rehab preferred
Excellent communication and critical thinking skills
Valid drivers license active auto insurance and CPR/BLS certification
