We are seeking a Physical Therapist (PT) for a contract assignment within a rehabilitation setting. The therapist will provide patient-centered care to individuals in skilled nursing and long-term care environments focusing on improving mobility function and overall quality of life. This is a rapid-response opportunity ideal for clinicians ready to start quickly.

Assignment Details:

Contract length: 13 weeks
Guaranteed hours: 36 hours per week
Shift: Days
Setting: Skilled Nursing Facility (SNF) / Long-Term Care

Key Responsibilities:

Evaluate patients and develop individualized treatment plans
Implement therapeutic exercises and mobility training
Monitor patient progress and adjust interventions as needed
Collaborate with interdisciplinary care teams
Maintain accurate clinical documentation

Requirements:

Active state Physical Therapist license (required)
Minimum 1 year of experience as a Physical Therapist
Experience in Skilled Nursing or Long-Term Care preferred
Strong clinical assessment and communication skills
