Job Title: Physical Therapist
Location: Syracuse NY (Client Confidential)
Contract Duration: 13 Weeks
Schedule: Day Shift Monday Friday
Hours: 8:00 AM 4:30 PM (40 hours/week)
Weekend Requirement: One weekend per month
Position Overview
A healthcare facility is seeking an experienced Physical Therapist for a 13-week contract assignment. The Physical Therapist will provide patient-centered care across acute and outpatient settings collaborating with interdisciplinary teams to develop and implement effective rehabilitation programs.
Key Responsibilities
Evaluate patients and develop individualized treatment plans in accordance with established procedures
Formulate specific measurable goals for patient treatment through collaboration with healthcare providers and patients
Implement therapeutic interventions to improve mobility function and quality of life
Document evaluations progress notes and discharge summaries accurately and in a timely manner
Participate in clinical rounds conferences and designated clinics
Communicate patient progress and care plans to the interdisciplinary team
Assist with discharge planning and patient education
Participate in staff education programs and professional development activities
Train and mentor new staff members as assigned
Serve as a clinical instructor for senior-level students when applicable
Support the development and implementation of new therapy programs
Required Qualifications
Bachelors degree in Physical Therapy
Active New York State Physical Therapist license (or license-eligible)
Minimum 2 years of Physical Therapy experience
Acute care and/or outpatient experience required
Ability to successfully pass pre-employment health clearance
