Schedule: MondayFriday 8:00 AM5:00 PM (occasional evenings to run a family group)
Compensation: $21.15$29.58 DOE and Licensure

What Youll Do as a Case Manager

• Complete comprehensive assessments and assist in creating individualized treatment plans
• Provide ongoing case management follow-ups and support to clients and their families
• Coordinate services referrals and community resources tailored to each clients needs
• Collaborate daily with therapists psychiatrists and other team members to ensure consistent high-quality care
• Participate in team meetings case reviews and treatment planning discussions
• Maintain clear timely documentation progress notes and care records
• Support clients in developing coping skills navigating challenges and working toward stability and recovery

Qualifications

Applicants must meet ONE of the following:

Note: Applicants under the working toward SSW pathway must obtain their SSW license within 24 months of hire.

• Associates degree or equivalent and working toward SSW licensure
• Bachelors degree with Social Service Worker (SSW) license
• Bachelors degree in any field with willingness to pursue SSW licensure

Additional Requirements:

• Valid drivers license
• Basic understanding of Social Work principles
• Familiarity with social welfare policy and human development
• Ability to problem-solve and maintain a client-centered approach
• Spanish speakers may qualify for increased wages after testing
