What Youll Do as a Case Manager
- Complete comprehensive assessments and assist in creating individualized treatment plans
- Provide ongoing case management follow-ups and support to clients and their families
- Coordinate services referrals and community resources tailored to each clients needs
- Collaborate daily with therapists psychiatrists and other team members to ensure consistent high-quality care
- Participate in team meetings case reviews and treatment planning discussions
- Maintain clear timely documentation progress notes and care records
- Support clients in developing coping skills navigating challenges and working toward stability and recovery
Qualifications
Applicants must meet ONE of the following:
Note: Applicants under the working toward SSW pathway must obtain their SSW license within 24 months of hire.
- Associates degree or equivalent and working toward SSW licensure
- Bachelors degree with Social Service Worker (SSW) license
- Bachelors degree in any field with willingness to pursue SSW licensure
Additional Requirements:
- Valid drivers license
- Basic understanding of Social Work principles
- Familiarity with social welfare policy and human development
- Ability to problem-solve and maintain a client-centered approach
- Spanish speakers may qualify for increased wages after testing
