Job Summary

Case Manager needed for the Grand Rapids area

JOB SUMMARY:

To provide nursing care to the terminally ill Hospice patient as needed. To provide assistance and understanding to the family in facility and home care situation and in time of bereavement. To work as a member of the Hospice team in providing Hospice care. Typical case load is 12-15 patients. Some on-call required.

QUALIFICATIONS:

  • Graduate from an accredited school of nursing.
  • Currently licensed as a Registered Nurse to practice in the state.
  • At least 1 year of nursing experience as a registered nurse.

KNOWLEDGE/SKILLS/ABILITIES needed to be successful in this role:

  • Ability to work independently make accurate and at times quick judgments.
  • Ability to respond appropriately to crisis outside of a hospital setting.
  • Must make judgments based on assessments and data available and act accordingly.
  • Must be flexible innovative and possess good interpersonal skills.
  • Must be able to cope with mental and emotional stress and demonstrate emotional stability.
  • Acceptance of and adaptability to different social racial cultural and religious modes.
  • Ability to complete Hospice training program.

TRANSPORTATION:

  • Must have a current valid drivers license auto liability insurance and reliable transportation.
  • Mileage reimbursed
