Occupational Therapist (Full-Time)
MondayFriday 8:00 AM5:00 PM

What Youll Do
Evaluate patients to identify occupational therapy needs and develop individualized Plans of Care
Complete initial evaluations reassessments and follow-up visits in compliance with OASIS and PPS guidelines
Implement and revise Plans of Care to ensure quality continuity and effective discharge planning
Communicate significant findings and changes in patient condition to physicians and interdisciplinary team members
Provide occupational therapy services in accordance with physician orders and accepted standards of practice
Instruct patients and caregivers in activities of daily living adaptive equipment use sensory integration and fine motor coordination
Assist patients and caregivers in obtaining necessary equipment including fabrication of custom splints when appropriate
Educate patients and families on home therapy programs and modify plans as needed
Supervise and provide clinical direction to COTAs HHAs and clinical staff to ensure high-quality care delivery
Promote patient safety using family and community resources
Physical & Work Requirements
Ability to sit stand bend lift and move intermittently
Ability to lift and transfer 50100 pounds
Willingness to travel within the assigned coverage area (up to 50%)
Home-based physically active work environment
May include occasional irregular hours or call coverage as applicable
What Were Looking For
Graduate of an accredited Occupational Therapy program
Current unrestricted OT license in the state of practice
Minimum one (1) year of experience in a clinical care setting
Reliable transportation valid drivers license and current auto insurance
Ability and willingness to travel within the branch coverage area
You will report to the Branch Director Clinical Manager Clinical Supervisor or designee and be supported by experienced leadership.
