Job description:
Responsibilities:
- Conduct assessments to evaluate patients physical emotional and cognitive abilities
- Develop individualized treatment plans based on assessment results
- Provide occupational therapy interventions to improve patients functional abilities
- Monitor and document patients progress and adjust treatment plans as needed
- Collaborate with other healthcare professionals to coordinate patient care
- Educate patients and their families on therapeutic techniques and strategies
Requirements:
- Masters degree in Occupational Therapy
- Valid state license or certification as an Occupational Therapist
- Previous experience in case management is preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Excellent problem-solving and critical-thinking abilities
