Position Purpose

At Mercy Care we are committed to serving Atlantas most vulnerable populations with compassion and dignity. As a Case Manager you will play a vital role in helping individuals and families navigate complex challenges by providing client-centered support and connecting them to essential services. This position is ideal for someone passionate about social justice advocacy and holistic care coordination.

What You Will Do

As a Case Manager you will:

  • Conduct comprehensive assessments to identify client needs strengths and barriers.

  • Develop individualized service plans with achievable short- and long-term goals.

  • Connect clients to housing healthcare employment behavioral health and other community resources.

  • Monitor progress through regular follow-ups and adjust care plans as needed.

  • Maintain accurate documentation in compliance with HIPAA and organizational standards.

  • Collaborate with internal teams and external partners to ensure continuity of care.

  • Advocate for clients to access and retain benefits.

  • Facilitate crisis interventions discharge planning and case conferencing.

  • Participate in team meetings supervision and ongoing professional development.

Minimum Qualifications

  • Education:

    • Required: Bachelors degree in Social Work Human Services Psychology Public Health or related field.

    • Preferred: Masters degree in a related field.

  • Certifications:

    • Required: CPR/First Aid

    • Preferred: Certified Case Manager (CCM) or equivalent.

  • Experience:

    • Required: At least 1 year in case management or social services.

    • Preferred: 23 years working with underserved populations such as the homeless elderly or those with chronic health conditions.
