Job Summary
CREST Collaborative is seeking an energetic LPN or RN to join our team who serves students in our medically fragile nurse is an integral part of the team which includes a classroom teacher support staff and related service include:Implement the plan of care for each individual student with chronic health conditions and with health care providers and other school personnel to provide an optimal level of care to each direct nursing care as well as participate in students activities of daily comprehensive documentation of delivery of care and student heath records in Electronic Health Record (EHR) using SNAP computer Medicaid documentation and submit collaboratively with the school nursing team to support needs across student programs.
