Empath Health is seeking an APRN to work in collaboration with Hospice Physicians the APRN Care Center & Home Teams (ACCHT) will assist with either Care Center or Home Team admissions and symptom management. The ACCHT will act as attending physician for patients who elect them as their attendings

What youll Do:

• Works primarily in a Care Center or with Facility-Based Home Teams but may provide care in multiple sites that include but are not limited to home hospice residence hospice inpatient unit acute care facility hospital based practice skilled nursing facility and clinics.
• Acts as attending physician for patients who make that choice at time of admission to hospice. This involves performing daily Care Center visits or necessary symptom management visits for Home Teams depending upon assignment.
• Assists Hospice Physicians in managing orders for patient admissions or managing calls for symptom management for existing patients.
• Performs comprehensive health assessments by obtaining medical histories and performing physical examinations.
• Diagnoses treats and monitors acute and chronic diseases in collaboration with a physician.
• Participates in the hospice 1^st on-call coverage schedule an average of one (1) weekend every four (4) weeks.

What Youll Need:

• Education: Masters Degree in Nursing or a related field.
• Licensure: Current Florida license as an Advanced Registered Nurse Practitioner certified in a practice are (if applicable to departmental requirements). Current DEA (Drug Enforcement Agency) license.
• Certifications: Valid CPR certification from the American Red Cross or American Heart Association.
• NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements).
• Current DEA License.
• Minimum of two (2) years nursing experience required.
• Minimum of two (2) year experience as an APRN required with experience in adult gerontology or oncology practice preferred.
