JOB SUMMARY:
As a Self-Directed Direct Support Professional you will work one-on-one with individuals to help them achieve the personal goals outlined in their service plan. Youll play an important role in fostering independence community inclusion and personâcentered support.
Depending on the service plan you may provide support in the individuals home in community settings or both. This position requires working independently while collaborating with the individual their broker and their circle of support. All duties must comply with Agency Program and NYS OPWDD guidelines.
JOB DUTIES AND RESPONSIBILITIES:
- Provide personâcentered support and services to individuals which may include personal care supervision home visits community activities protective oversight and related supports.
- Teach skills needed to achieve goals identified in the individuals plan and provide documentation to support goal evaluation.
- Follow guidelines and instructions from the Broker and the individual/circle of support to meet the persons specific needs; adhere to OPWDD Core Competency expectations for Direct Support Professionals
- Participate in and implement the Staff Action Plan make recommendations and offer ideas to enhance community inclusion
- Submit daily documentation in compliance with Medicaid requirements using eVero software email and related tools.
- Provide safe and reliable transportation
- Follow all applicable organizational policies and procedures; document and report incidents in accordance with OPWDD regulations and Agency procedures.
- Maintain required paperwork accurately thoroughly and in a timely manner per state program and Agency requirements.
- Maintain confidentiality of all information.
- Participate in required orientation and annual trainings.
JOB REQUIREMENTS:
- Ability to work with others in a warm professional manner and be a positive role model to individuals.
- Must safeguard the individual show respect for and promote the individuals dignity and follow goals identified in the plan.
- Ability to work assigned schedule and be reasonably flexible.
- Demonstrates OPWDD Core Competencies and follow OPWDD Code of Ethics.
- Must have the ability to read and comprehend instructions correspondence and have the ability to write simple correspondence.
- Must have basic phone app/computer skills to adequately document notes within Evero Software sumbit timesheets email etc.
- Compliance with documentation and other requirements daily.
- Basic reasoning skills including the ability to analyze and problem solve.
- Must possess a valid New York State drivers license with good driving record in accordance with Agency policy and procedure and have access to a reliable and insured vehicle.
- Must be able to tolerate a moderate noise level in the work environment handle distractions while performing this job may be required to sit stand use hands reach talk hear walk or lift up to 50 pounds.
- Must successfully complete all Agency and program required orientation/trainings within 30 days of hire and maintain required certifications and trainings for skill enhancement.
MINIMUM QUALIFICATIONS:
- High School Diploma or G.E.D preferred applicable job experience preferred.
- 1-3 years related paid or life experience with special populations.
