Garden Gate Skilled Nursing & Rehab is seeking a reliable and experienced Licensed Practical Nurse (LPN) Unit Coordinator to lead a dedicated nursing unit. This is an excellent opportunity for an LPN who is ready to step into a leadership role and make a meaningful impact on both resident care and team development.

Key Responsibilities:

Coordinate daily clinical operations for the assigned unit
Provide hands-on nursing care and oversee medication and treatment administration
Supervise and support CNAs and unit staff to ensure high-quality resident care
Monitor documentation care plans and compliance with regulatory standards
Serve as a point of contact for families physicians and interdisciplinary team members
Participate in staff scheduling training and performance discussions
Support communication between nursing leadership and frontline staff

Requirements:

Current LPN license
Minimum of 1 year experience in long-term care or skilled nursing (Preferred)
Strong leadership organizational and communication skills
Ability to effectively delegate problem-solve and prioritize care
Experience in a lead nurse charge nurse or team leader role preferred
