Traveler shall be scheduled in accordance with facility policy and is expected to work weekends if needed. 1 experience acceptable Hospital: Fairview Park Hospital Location: Fairview Park Therapy Center/ Hospital Address:200 Industrial Blvd Unit: Outpatient therapy(Fairview Therapy Center)/acute care therapy(6th floor) Building: Outpatient therapy(Fairview Therapy Center)/Acute Care Hospital(6th floor therapy room) General Information Therapy areas both acute and outpt Specifics: Number of Beds: 190 Patient Ratios: Outpt therapy: 1 pt per hour-8:00-5:00; closed 1 hour for lunch; Acute care: 8:00-4:30 with 30 min lunch-patient 10-14 caseload Therapy staffing description- (unit secretaries and tech availability): outpatient therapy has a office coordinator registrar and a outpt therapy coordinator; acute care staff work floors of hospital 3rd 4th 6th-have a room on 6th floor where supplies are located documentation can be done Type of equipment: all modalities ARJO lifting equipment Aquaciser UE bike fine motor coordination act strengthening equip cardio equipment Documentation system: Redoc-outpt; meditech-acute care Scheduling requirements: Monday-Friday; may have to do a Saturday for coverage-if so will have a day off during week; 8 hours/day Floating policy: Patient Experience requirements Outpatient therapy and acute care experience preferred Hospital Specifics- Certifications: Hospital is stroke joint sepsis heart Hiring profile Years of Experience Required: at least a year preferred Requested Skill Set: Physical Therapist/Physical Therapist skills Must have: Procedures performed in therapy areas: therapeutic activities therapeutic exercises modalities gait training kinesiotaping aquaciser neuromuscular reeducation other PT/PTA procedures Certifications Required: CPR-American Heart Association Dress Code: (Required uniform) Teal scrubs or teal collared shirt with kakhi black gray pants Unit Guidelines/Policies: Please attach unit specific policies and guidelines. In the outpatient therapy area we treat patients one to one for 1 hour. We offer vestibular rehab lymphedema aquatic general therapy services. We treat pediatric to geriatric. We have a comprehensive team of professionals who work together for the best interest of the our acute therapy area we work side by side other healthcare professionals. We have outstanding interdisciplinary communication disciplines working together to give the patient the best care. We do instruction and training with PCT employees on transfer training use of gait belt to focus on patient and employee safety.
