Position Summary
The Social Worker / Integrated Student Support Specialist plays a critical role in supporting students academic behavioral and social-emotional success through school-based interventions and strong homeschoolcommunity partnerships. This position provides direct student services coordinates Section 504 Plans and connects families with internal and external resources to ensure equitable access to support.
Key Responsibilities
Student Services & Support
Provide regular individual check-ins for students requiring social emotional or behavioral support.
Design implement and facilitate counseling groups (e.g. social skills anger management trauma response self-regulation attendance grief).
Collaborate with school-based teams to identify students in need of targeted or intensive interventions.
504 Plan Case Management
Serve as the case manager for students with Section 504 Plans.
Coordinate evaluations accommodation planning annual reviews and implementation in compliance with state and federal law.
Consult with teachers and staff to support effective delivery of accommodations.
Family & Community Engagement
Conduct home visits to support attendance family communication crisis response and wraparound planning.
Engage families in collaborative problem-solving to reduce barriers to student success.
Serve as a liaison to community agencies and external service providers (mental health housing medical etc.).
MTSS Participation
Participate in Tier 2 and Tier 3 MTSS processes including attendance behavior and emotional wellness teams.
Assist in developing and monitoring attendance improvement plans and behavioral support plans.
Document interventions and monitor student response to supports.
Compliance & Documentation
Maintain accurate confidential records of services provided including check-ins groups 504 meetings and home visits.
Track service data and outcomes for planning and program evaluation.
Ensure compliance with district state and federal regulations including mandated reporting and student privacy requirements.
Professional Development & School Capacity Building
Support staff learning related to trauma-informed practices restorative approaches student engagement and cultural responsiveness.
Contribute to initiatives focused on improving school climate and student wellness.
Qualifications
Required
Masters degree in Social Work Counseling School Psychology or a related field
Valid Pennsylvania Educational Specialist certification or PA professional licensure (LSW LCSW LPC or equivalent)
Experience with student case management counseling services and 504 Plan coordination
Knowledge of Pennsylvania laws related to 504 Plans school attendance and child welfare
Valid Pennsylvania drivers license and reliable transportation
