In this role youll perform essential LPN duties within long-term care settings: conducting comprehensive patient assessments and vitals monitoring administering prescribed medications and injections evaluating responses and potential adverse effects and updating care plans in collaboration with RNs and physicians. Youll assist with activities of daily living support mobility and rehabilitation efforts manage wound care and dressing changes prevent falls and help with nutrition and hydration management. Your documentation in electronic health records will be precise and timely ensuring continuity of care across shifts and interdisciplinary teams. Youll also contribute to care conferences mentor nursing assistants and pursue opportunities to deepen your clinical skills in geriatrics palliative care and chronic disease management.
