$25.67 - $37.70

Job Description:

Purpose of Job

Improve the overall health of the communities we serve by training highly-motivated clinical pharmacists so that they are prepared to independently and confidently provide advanced patient care services in the ambulatory and managed care settings. Upon completion of the program the resident will demonstrate proficiency in: collaboration with other healthcare providers providing pharmaceutical care to diverse patient populations and completion of a teaching certificate. Duties of this position are as follows:

Essential Duties and Responsibilities

  • Checks medication orders for therapeutic appropriateness; checks transcription of medication orders; updates medications on administrative records; observes for adverse drug reactions; provides medication counseling to patients; provides drug information to physicians nurses patients and other health care professionals; documents interventions with health care personnel related to drug therapy; performs drug utilization evaluations on medications; attends educational seminars.

  • Rounds with clinical pharmacists to gain working knowledge of various disease states.

  • Coordinates with pharmacy leadership team to identify longitudinal project needs for organization including design implementation and presentation of project status.

  • Completes the Washington State University College of Pharmacy Teaching Certificate during the residency year.

  • Assists in precepting pharmacy students during clerkship rotations.

  • Performs other duties assigned including supporting the CHAS Health Mission and Core Values.

Qualifications

Education/Experience: PharmD from accredited school of pharmacy and valid Washington State pharmacy intern license at time of hire.

Skills: Excellent oral and written communications skills required. Computer skills required. Selfdirected independent motivated learner required. Ability to maintain confidentiality. BLS (CPR/AED) required. Commitment to supporting a safe and respectful environment is required.

Physical Demands

Pharmacists must be able to move around the facility between 1/3rd and 2/3rds of the day sitting at a workstation occurs under 1/3rd of the day. Using hands and reaching with arms occurs over 2/3rds of the day. Communication is required over 2/3rds of the day. Climbing stooping or crawling is minimal. Lifting up to 10 lbs occurs occasionally while 25-50 lbs occurs infrequently.
