Requirements

Essential Qualifications

Licensure: Must hold a current Doctors license from the State of Texas Medical Board.
Residency: Successful completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
Certifications:

Current Drug Enforcement Agency (DEA) registration certificate.
National Provider Identifier (NPI#)
