Coverage Location: St. Ritas Cancer Center Lima Ohio & Grand Lake Regional Cancer Center Celina Ohio

Practice Setting: Outpatient & Free-Standing Cancer Centers

Reason for Coverage: Physician Retirement

Dates of Coverage: 6/1/2026-12/31/2026

Schedule: M-F 8a-4p

Call Schedule: Permanent providers will handle

Number of Physicians in Practice: 1

Will There be Other Radiation Oncologists Present During Locum Tenens Coverage: Yes

How Many APPs Provide Coverage: 1

Average Number of Patients Per Day - OUTPATIENT: 20

Average Number of Patients Per Day - INPATIENT: 1

Trauma Level/Designation (Hospital): 2

Patient Population for Required Coverage: Adult Hospital Based Office/Clinic

Documentation System/EMR: Mosaiq Epic

Required Procedures:
Conventional Radiotherapy 3D Radiotherapy MRT External Beam Radiation - 3D-CRT External Beam Radiation - IGRT External Beam Radiation - IMRT Stereotactic Radiosurgery Stereotactic Body Radiation Therapy (SBRT) Fractionated Stereotactic Radiotherapy

Required Duties:
Orders Tests Orders Images Prescribes

Estimated Credentialing Timeframe: 90-120 days

Board Certified - REQUIRED
OH license (willing to accept those who will obtain) - REQUIRED
BLS - REQUIRED
A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - REQUIRED
