Job Summary

  1. Provide assistance with activities during assigned shift at the Supervised Living Home.
  2. Assist with an assessment of individuals strengths and needs.
  3. Assist with preparing meals for the individuals and provide client training as recommended.
  4. Promote and encourage the independence of each individual in the Supervised Living Home.
  5. Follow the proper procedures when reporting a serious incident to the Site Manager.
  6. Ensure the safety of each individual in the Supervised Living Home at all times.
  7. Follow all Millcreek HCBS waiver Policies and Procedures
