Home Health Clinical RN Director of Nursing - Empath Home Health (Naples Sarasota Fort Myers)

Empath Home Health a part of Empath Health is seeking a caring and motivated RN DON to work with our Home Health Division for the following locations: Naples Sarasota & Fort Myers. You are responsible for the supervision and management while demonstrating fiscal responsibility.

Schedule: Monday Friday 8:00 AM- 5:00 PM.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home along with personal care support that assists with daily needs as part of a seamless continuum of care. What youll Do

• Develop plan implement analyze and organize clinical operations for the care center.
• Review requests for services determine patient eligibility and coordinate patient referrals and admissions with the intake team.
• Oversee scheduling activities to ensure patient needs are met reviewing assignments with the patient services coordinator and professional/support staff.
• Oversee payor verification and pre-certification requirements communicating with payors and their representatives. Review documentation of other staff members ensuring appropriateness and resolving issues.
• Assist clinicians in establishing immediate and long-term patient goals developing Plan of Care (POC) and overseeing the management and delivery of patient care.

What Youll Need

• Associate or bachelors degree in nursing.
• One (1) year of supervisory and/or management experience as an RN in a healthcare environment.
• One (1) year of RN experience in a Home Health or Hospice environment.
• Current CPR certification.
• Current and unencumbered license to practice as a Registered Nurse Licensed Vocational/Practical Nurse specific to the state the employee is assigned to work by the company.
• Valid Florida license as a Registered Nurse
