Job Summary

The Geriatric Clinical Pharmacist is a Doctor of Pharmacy (PharmD) who is responsible for supporting geriatric clinical pharmacy practices at Jefferson House (JH) and Cedar Mountain Commons (CMC) by applying advanced knowledge clinical and research skills. The Geriatric Clinical Pharmacist provides Comprehensive Medication Management CMM services including prospective Medication Reconciliation MR and Medication Therapy Management MTM for JH patients care transitions patients and CMC Residents. The geriatric clinical pharmacist in consultation and collaboration with JH/CMC clinical operations and HH Research staff will develop implement coordinate and evaluate pharmacy projects and programs for members of the clinical team the patient populations and communities we serve. This is accomplished to ensure role competence compliance with regulatory standards and to meet organizational goals. These services will target the CMS GǣTriple AIMGǥ by improving quality of care reducing adverse drug effects ADEs preventing medication related problems MRPs and helping reduce preventable ER visits and hospital readmissions.

  • Develops and coordinates curriculum/plan utilizing principles of age-appropriate learning and effective use of resources (human material and technological) to meet team assesses learning needs for geriatric pharmacy clinical and regulatory outcomes
  • Establishes collaborative professional relationships with key stakeholders inclusive of Medical Director Primary Care Providers Medical Specialists (cardiology and pulmonary services) Director of Nursing Registered Nurses Social Workers Case Managers Rehabilitative Specialists HH Pharmacy Department Community Pharmacy Providers and Administrative Assistants
  • Conducts audit of pharmacy operations policies procedures to identify opportunities for improving efficiency quality of care reducing medication related problems adverse drug effects and cost.
  • Evaluates use of standardized order sets medication administration protocols and formulary management/utilization tools
  • Evaluates medication storage system solutions for best-practices and opportunities to improve efficiencies and reduce costs (e.g. PYXIS Talyst medication carts)
  • Develops Short Term Rehabilitation medication reconciliation protocols on admission/discharge that address reducing polypharmacy target medication-related activities of daily living patient/family education and reducing medication complexity
  • Creates Care Transitions protocols for identifying high-modifiable risk discharge patients who would benefit from a pharmacist intervention at discharge (may include patients with complex medication therapies history of multiple readmissions high-risk medications (HRMs) potentially inappropriate medications etc.)
  • Establishes protocol for post-discharge in-home pharmacy assessment of above patients
  • Provides medication therapy management consults for LTC/STR patients. Develop clinical reporting metrics for LTC as required by CMS.
  • Collaborates with staff to promote non-pharmacologic management of behavioral and psychological symptoms of dementia (BPSD)
  • Promotes patient/family education programs that address medication adherence safety and awareness
  • Provides real-time drug information services for JH/CMC clinical operations
  • Promotes practice standards to ensure optimal patient-focused outcomes team competence and compliance with CMS standards
  • Collaborates/Partners with JH/CMC clinical staff to provide optimal pharmacy services for team members that supports the organizations standards of care and framework for professional practice
  • Collaborates with JH/CMC Affiliates to facilitate patient care staff training development and education in order to ensure consistency continuity and cost-effectiveness across the continuum and system
  • Maximizes the potential of the JH/CMC team by documenting patient outcomes pharmacy process/procedures audits and revising based on results
  • Identifies implements and evaluates various strategies to enhance use of evidence based best practices as part of daily work flow(s) for the JH/CMC clinical team
  • Provides Clinical Leadership to model professional practice Hartford Healthcare Leadership behaviors and values into daily works

  • Develops and maintains geriatric pharmacy educational competencies for JH/CMC team roles
