What youll bring:
Masters Degree or Bachelors Degree in Social Work or related Social Service Field required.
Experience in a medical or hospital setting or discharge planning preferred.
Current CPR per assigned area(s) may be required.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact professionalism and diplomacy.
Excellent organizational skills detail-oriented a self-starter possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Job Specific Duties:
As applicable to assigned area(s); Addresses identified support and social needs of patients including but not limited to: trauma palliative care comfort care/end of life patients patients with substance use disorder patients with complex social situations patients with active psychiatric and/or behavioral concerns situations with Guardianship and other legal issues impacting health and recovery; Supports patients and families in making difficult decisions related to hospital stay and discharge.
Exhibits attentiveness of risk for readmission and works with the interdisciplinary team across the healthcare continuum to address social determinants of health that may impact the discharge planning process. Keeps the patient and identified support person(s) engaged in all aspects of planning.
As applicable to assigned area(s); Partners with appropriate stakeholders across the healthcare continuum to identify social barriers to discharge; Acts as an expert regarding community resources healthcare needs impacting discharge plan and social barriers to discharge; Remains mindful of registration status and payer requirements as they relate to discharge options in order to fully support patients and families in making informed healthcare decisions.
Explores and communicates solutions and recommendations to social barriers impacting healthcare with appropriate stakeholders Seeks input from the multidisciplinary team and remains aware of the discharge plan. Completes thorough and accurate documentation in the patients medical record.
As applicable to assigned area(s); Performs as an active member of the healthcare team making early referrals and communicating thoroughly with post-acute partners family members and patients; Operates from a person-in-environment framework and strength-based perspective; Exhibits strong professional communication skills and ensures available community connections are made prior to discharge.
Exhibits knowledge and skills necessary to provide for the physical psychological and cultural needs of the patient. Advocates for clients rights to self-determination confidentiality and access to supportive services and resources and appropriate inclusion in informed decision-making that affects health and well-being.
Annually completes one (1) organization-wide teaching event or one (1) department-wide training session specific to Social Work.
In collaboration with leadership identifies and assumes responsibility of one (1) existing or needed Case Management policy that is specific to Social Work to review and update annually.
Consistently reflects ownership and professionalism towards Care Coordination. Identifies areasin need of improvement and serves an active role in developing the Organizations Core Values to both internal and external customers.
Responds to and provides direction in a changing workload in a positive and professional manner.

Shift:
Variable (United States of America)Schedule: PRN scheduled as needed. Variable day shifts may be 8 hours or 10 and could be Monday - Sunday.
Logan Health operates 24 hours per day seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
PRN positions (also referred to as Casual As Needed or Per Diem) at Logan Health offer flexibility and are designed to provide coverage during busy periods employee absences and vacations making them an excellent opportunity to support the team as needed. PRN employees can participate in a matching 401(k) plan and have access to the Employee Assistance Program. Additionally they may qualify for hourly differential pay for certain shifts or hours offering extra compensation based on department-specific scheduling needs.Pay rates and scheduling practices may vary by department.
