Purpose of this position:
The purpose of the MDSCoordinator is to develop and complete the patient assessment process inaccordance with the requirements of federal and state regulations. The MDS Coordinator is to complete minimumdata set (MDS) assessments and create comprehensive plans of care after reviewof the patients medical record and communication with direct care staff thepatients physician and family. Theyalso promote understanding of the aging population.
Qualifications:
- Valid LPN or RN license from the Virginia State Board of Nursing.
- Prefer to have a current/active CPR certification.
- Personalintegrity and professionalism to work effectively with other team membersresidents and family members.
- Working knowledge of State and Federal laws regulations and guidelines as itpertains to Longterm care facilities.
- Preferred RAI experience.
- Preferred proficiency in ICD 10 coding.
- Preferred experience in developing comprehensive care plans.
