Overview
Provide patient assessment teaching and follow up instruction for adult patients with diabetes in the outpatient clinic setting (New London Hospital and Newport Health Center).
Responsibilities
- Collaborates with primary care providers and teams to support patients with diabetes and their families/caregivers.
- Serve as coordinator of the American Diabetes Association Diabetes Self-Management Education and Support (DAMES) Program. Collect required data and report as required.
- Serve as a coordinator of the CDC - National Diabetes Prevention Program (National DPP). Facilitate collect required data and report as required.
- Coordinate and oversee the planning implementation evaluation and effectiveness of the diabetes education program in collaboration with the primary care teams.
- Actively participate in quality initiatives related to care of diabetes patients.
- Act as a liaison to D-HH for purpose of the ADA-DSMES program.
- Other duties as assigned.
Qualifications
- Bachelors Degree in Nursing preferred
- 3-5 years experience as a Certified Diabetes Educator in the ambulatory setting
Required Licensure/Certifications
REQUIRED LICENSE(S):
Licensed RN with NH eligibility
REQUIRED CERTIFICATIONS:
Certified Diabetes Educator (CDE)
