Job Summary and Qualifications
The Manager Case Management reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department. The Manager for Case Management is accountable for the overall day-to-day oversight and management of the case management program including coordination supervision and administrative oversight of the case management team. This position also functions as an expert clinical practitioner case management subject-matter expert resource advisor and leader for the members of the case management team. The Manager for Case Management supervises and monitors professional and support staff and ensures that effective care coordination and case management practices are consistent with hospital policies and applicable regulations and guidelines. The Manager for Case Management monitors quality of the interactions documentation and assessments of the case management staff. The Manager for Case Management coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
What qualifications you will need:
Bachelors Degree in Nursing OR Licensed Clinical Social Worker with Masters Degree in Social Work required
3 years experience in clinical nursing in an acute care setting OR hospital social work required
2 years of supervisory/leadership experience required in an acute care setting
InterQual experience preferred
Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation OR currently licensed as a LCSW or a LMSW in the state(s)of practice according to law and regulation required
Certification in Case Management and/or Certification in InterQual or IQCI preferred
Additional Information:
Case Management required for the entire hospital (612 bed academic medical center with a freestanding childrens hospital)
