Job Description

Key Responsibilities

Conduct in-home medical and psychosocial assessments
Evaluate client needs and recommend appropriate services
Develop individualized care plans focused on independence
Coordinate services and refer clients to community resources
Advocate for client rights and self-determination
Complete accurate documentation in electronic medical records
Complete a minimum of 17 assessments per week (appointments scheduled for you)
