- RTO will not be approved in the first week of assignment or holidays.

- All shifts the traveler is open to working on should be listed on the submission.
- All departments the traveler is open to working must be checked on the cover page.
- All candidates submitted should be preclosed and ready to accept.
- Please let us know if the traveler withdraws or is no longer available.
Requirement description :
- Candidates with an AZ drivers license will not be considered - REQUIRED
- Provider must have 24 months minimum of current Progressive Care Unit/PCU experience - REQUIRED
- Certifications must be American Heart Association or American Red Cross - REQUIRED
- Nursys cannot show any hits on license at time of submission - REQUIRED DOCUMENT
- Banner will not approve RTO within the first week or holidays. Can not exceed 7 days - REQUIRED
- Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**
