Position Overview:

The Neurophysiologist Physician provides real-time remote intraoperative neuromonitoring interpretation and clinical oversight for Veterans undergoing complex surgical procedures at the Bruce W. Carter VAMedical Center. This role includes continuous analysis of neurophysiologicdata supervision of onsite IONM technologists and communication of clinicallysignificant changes to the surgical and anesthesia teams and adherence to
VA privileging and national standards of care.

    • Availability for scheduled IONM cases typically 3 days per week during BWCVAMC OR hours with case times typically between 6:30 a.m. and 4:30 p.m. EST.
    • 24/7/365 on-call coverage for remote intraoperative monitoring and phone consultation including evenings weekends and federal holidays; must be reachable within 15 minutes by phone for on-call consults.

Qualifications

  • Education: MD or DO degree with completion of an accredited neurology/ neurosurgery residency and subspecialty training or fellowship in intraoperative neurophysiological monitoring or a closely related field.
  • Licensure:
    • Current full and unrestricted license to practice medicine in any U.S. state territory or the District of Columbia; no history of restricted suspended revoked voluntarily surrendered pending action or denied license.
    • Current BLS and ACLS certifications (or equivalent); ongoing CME in clinical neurophysiology/IONM sufficient to maintain board status and privileging.
    • Board Certification by the American Board of Neurological Surgery (ABNS).
  • Experience: Fellowship or specialized training in intraoperative neurophysiological monitoring with at least 2 years of experience providing comprehensive IONM supervision during surgeries.
  • Credentialing Requirements:
    • Eligibility and successful completion of VA credentialing/privileging (VetPro NPDB query state board checks) and compliance with all infection control occupational health and mandatory training requirements as for other clinical staff.
    • Must comply with all immigration nationality and E-Verify requirements and must not appear on the HHS OIG List of Excluded Individuals/Entities.
    • Requires successful VA credentialing and privileging (VHA Directives1100.20 and 1100.21) before providing services with no history ofrestricted suspended revoked or voluntarily surrendered license.
    • Provider functions as an independent contractor (non-VA employee) and must maintain own malpractice coverage and compliance with allconflict-of-interest citizenship and OIG exclusion requirements.
  • Skills:
    • Expert level competence in the interpretation of multimodal IONM data (SSEP MEP EMG EEG) and correlation with surgical anatomy and pathophysiology.
    • Strong decision-making and communication skills to provide clear timely recommendations to surgeons and anesthesia providers in high-risk intraoperative situations.
    • Ability to supervise and coach IONM technologists ensuring adherence to protocols troubleshooting complex signal issues and optimizing data quality.
    • Familiarity with VA clinical systems including electronic documentation standards privacy requirements and patient safety reporting expectations.
    • Commitment to continuous quality improvement data-driven practice and collaboration with VA leadership and COR to meet QASP performance metrics and CPARS expectations.

Key Responsibilities

  • Provide remote real-time intraoperative neurophysiologic monitoring interpretation for VA patients undergoing spine neurosurgery vascular cardiac thoracic orthopedics ENT and interventional neuroradiology procedures.
  • Supervise IONM technologists and direct selection of appropriate monitoring modalities (e.g. SSEP MEP EMG EEG) and stimulation/recording parameters based on surgical procedure and
    patient factors.
  • Continuously review incoming neurophysiologic data identify significant changes suggestive of neural compromise and promptly communicate findings and recommended interventions to the surgical and anesthesia teams.
  • Document intraoperative interpretations events and recommendations in alignment with VA EHR documentation standards and local privileging requirements.
  • Participate in pre-operative planning and case review when required including assessment of prior studies risk factors and optimal monitoring strategy.
  • Engage in post-case review quality improvement activities and respond to COR and VA clinical leadership regarding clinical performance incident reviews and CPARS/QASP-driven feedback.
  • Ensure practice complies with VA policies VHA directives (e.g. patient advocacy test result communication credentialing/privileging) and federal privacy and security statutes.
  • Physical Examination Responsibilities:
    • Review relevant preoperative clinical information (neurologic status imaging labs as appropriate) to inform monitoring strategy and interpret intraoperative changes in context.
    • Provide intraoperative assessment of neural function via interpretation of neurophysiologic signals and advise the surgical team on clinical significance and recommended actions.
  • Medical Oversight Responsibilities:
    • Provide overall technical and clinical oversight of IONM services as the supervising Neurophysiologist for technologists in the OR.
    • Ensure all monitoring is delivered within the providers approved scope of practice and VA-granted privileges and in accordance with AANS ABNM ASNM ABRET and Joint Commission standards.
    • Participate in incident reviews patient safety evaluations and performance improvement plans when patient safety events or quality concerns involving IONM occur.
