Position Summary

The organization is seeking a dedicated and compassionate Nurse Practitioner (NP) to join its full-time nursing home and house call team in Brooklyn NY.

This is a full-time outpatient-focused role providing medical care to patients in skilled nursing facilities and home-based settings. The Nurse Practitioner will manage acute and chronic conditions for elderly and medically complex patients with a strong focus on continuity of care preventive services and reducing unnecessary hospitalizations.

Essential Position Responsibilities

  • Provide comprehensive medical care to patients in nursing homes and home-based settings

  • Perform history and physical examinations routine follow-ups and urgent evaluations

  • Diagnose and manage acute and chronic medical conditions

  • Order interpret and evaluate diagnostic tests and labs

  • Develop and manage individualized treatment and care plans

  • Coordinate care with primary care physicians specialists nursing staff and caregivers

  • Educate patients and families on disease management medications and preventive care

  • Maintain accurate and timely documentation in the electronic health record (EHR)

  • Participate in care coordination quality improvement initiatives and population health efforts

Qualifications

  • Education: Graduate of an accredited Nurse Practitioner program (MSN or DNP)

  • Licensure & Certification: Active or eligible New York State Nurse Practitioner license; national certification required (FNP AGNP or GNP preferred)

  • Experience: Prior experience in nursing home house call geriatrics or primary care preferred; new graduates are welcome to apply
