Position Summary: We are seeking a highly skilled dual board-certified Family Nurse Practitioner (FNP-C FNP-BC) with extensive experience in Emergency Medicine Primary Care Gastroenterology and Telehealth services. The ideal candidate is confident in autonomous clinical decision-making thrives in high-acuity environments and collaborates effectively with interdisciplinary teams to deliver safe evidence-based and patient-centered care.
This role supports both acute and chronic patient populations across emergency outpatient and virtual care settings while contributing to quality improvement and operational excellence initiatives.
Key Responsibilities: Clinical Care

Perform comprehensive assessments diagnosis and management of acute and chronic conditions across lifespan

Provide emergency stabilization and extended observation care for high-acuity patients

Deliver same-day urgent and preventive primary care services
-
Manage complex gastrointestinal and hepatobiliary disorders in collaboration with specialists
-
Prescribe medications order/interprete labs and imaging and develop treatment plans
-
Conduct telemedicine visits for acute and follow-up care
Emergency & Acute Services

Manage patients in Level I Trauma and high-volume emergency settings
-
Triage prioritize and coordinate patient flow
-
Stabilize critical patients and coordinate admissions or specialty referrals
-
Apply evidence-based protocols to reduce unnecessary admissions and improve outcomes
Care Coordination

Collaborate with physicians residents consultants and multidisciplinary teams
-
Coordinate specialty referrals and transitions of care
-
Educate patients and families on disease management and preventive care
Documentation & Systems
-
Utilize Epic and specialty EHR systems for accurate documentation
-
Maintain regulatory compliance and quality standards
-
Track patient outcomes and support performance improvement initiatives
Leadership & Education

Lead quality improvement and patient experience initiatives
-
Participate in unit practice councils and staff development programs
-
Precept nursing students and mentor new clinicians
