The idealRN Case Manager (RNCM)is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life this role you will collaboratively works alongside an interdisciplinaryteam to provide end-of-life care to those needing hospice services in their home or community setting. You will provide individualized case management admission facilitation clinical support continuous care after hours call and/or other skilled nursing expertise with chronic and end-of-life care.
Position Type:Full-time benefits eligible position
Shift Available: 8:00am 5:00pm Monday Friday
Wage Range:$83000 - $95000 annual salary
RN Case Manager Responsibilities:
- Succinctly convey patient assessments with clinical data to hospice physician who will then determine hospice eligibility
- Provide direct patient care to terminally ill patients; assess identify and address their individual physical psychosocial safety and comfort needs
- Anticipate prevent and treat undesirable symptoms or secondary symptoms
- Develop maintain and manage an individualized care plan that captures the evolving needs of the terminally ill patient
- Train instruct and support the patient family and caregiver
- Document patient problems nursing assessments patient goals care provided and patient/family outcomes from interventions and care provided in a timely manner
- Consult and collaborate respectfully with the interdisciplinary team and others involved in the patients care
- Prepare and present all needed information for bi-weekly IDG meetings
- Graduate of accredited school of nursing
- Current state of Minnesota RN licensure
- Three years recent nursing experience in acute long-term care or home care/hospice/public health
- Patient care planning and patient teaching skills
- Critical thinker with excellent written and verbal communication skills
- Current Minnesota drivers license insurance and daily access to reliable vehicle
