Job Summary

Pay Range:$52.19 - $52.19 per hour

Status: Full-time (35 hours per week)

FLSA Classification:Non-exempt

Role Summary: The Licensed Practical Nurse (LPN) provides essential nursing support in a transitional housing program serving clients with mental health and stabilization needs. Working collaboratively with case managers social workers psychiatrists and program staff the LPN ensures client well-being through assessment medication management crisis response and care coordination. Responsibilities adapt to program needs and prioritize client safety dignity and continuity of care.

Primary Job Responsibilities/Duties:

The primary job duties and responsibilities of the Licensed Practical Nurse shall include but not necessarily be limited to the following:

  • Conduct patient assessments during routine interactions identifying physical behavioral and psychological needs.
  • Build trusting relationships with clients by actively listening interpreting concerns and responding with empathy.
  • Verify client identity and ensure accurate medication administration in compliance with program protocols.
  • Respond calmly and effectively to distressed or escalated clients using de-escalation techniques.
  • Identify risks of self-harm or harm to others and initiate appropriate safety interventions.
  • Maintain secure medication storage units and accurate inventory records.
  • Prepare and update client records including progress notes care plans and discharge summaries.
  • Develop individualized care plans in consultation with psychiatrists case managers and social services staff.
  • Coordinate client transitions out of the shelter including after-care follow-up with case managers.
  • Participate in team meetings case conferences and external consultations as required.
  • Foster respectful collaborative relationships with colleagues to support high-quality integrated care.
  • Continuously seek innovative approaches to improve service delivery and client outcomes.
  • Communicate clearly and professionally in written and verbal forms adhering to ethical legal and documentation standards.
  • Manage time resources and competing priorities effectively in a fast-paced environment.
  • Maintain clinical expertise in acute mental health care including suicide risk assessment and crisis management.

Physical Requirements:

  • Ability to sit stand or walk for extended periods; perform repetitive tasks (e.g. documentation).
  • Lift and carry up to 25 pounds.
  • Travel between NAICA sites across NYC as needed.
  • Navigate all areas of program facilities including stairs and common spaces.

Work Environment / Schedule Requirements:

  • May include evening overnight weekend or holiday shifts with applicable differential pay.
  • Staff must remain on duty until properly relieved or authorized to leave by a supervisor.
  • Role involves exposure to emotional intensity crisis situations and clients experiencing mental health challenges.
  • This position follows a regular weekly schedule. However the Program operates twenty-four (24) hours per day and three hundred sixty-five (365) days per year so variable or additional hours may occasionally be necessary to effectively fulfill the responsibilities of the position based on organizational needs deadlines or events.

Qualifications:

  • Active New York State LPN license.
  • Minimum five (5) years of nursing experience in mental health settings.
  • Bilingual in English and Spanish or English and French.
  • Mental Health Nursing Certification is a plus
  • Experience in homeless services transitional housing or crisis intervention is preferred.
  • Must be able to read speak write and understand English for administrative purposes.
  • Must pass drug screening to be appointed.
  • This position may be subject to a series of investigations before and after appointment.
