Job Summary:
This position is for a Licensed Vocational Nurse (LVN/LPN) in a home health setting responsible for providing nursing care under the direction of a Registered Nurse. The role involves gathering and reporting patient data carrying out orders and performing therapeutic procedures appropriate for the patients age and condition. The LVN/LPN will work in West Portland OR and surrounding areas including Beaverton with a focus on minimizing drive time between patient visits.

Responsibilities:
- Providing appropriate nursing care as directed by an RN.
- Gathering and reporting patient data.
- Carrying out physician orders.
- Performing therapeutic procedures on patients.
- Documenting patient care and related duties.
- Assisting patients with activities of daily living.
- Collecting specimens.
- Obtaining and recording vital signs.
- Communicating patient status and reporting changes in condition to licensed professionals.
- Maintaining and cleaning equipment.
- Managing patient care using EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis.
Required Skills & Certifications:
- Active LVN/LPN license in the state of Oregon.
- BCLS certification.
- Minimum of 1 year of skilled home health experience.
- Valid drivers license car insurance and reliable transportation.
- Oasis experience.
- COVID-19 vaccination primary series and one booster.
Preferred Skills & Certifications:
- Experience in emergency or critical care environments.
- Comfort level working with vents/trachs/PICC lines.
- 2 years of experience in direct patient care.
Special Considerations:
- Traveler is expected to have their own transportation.
- Facility can call off UP TO one shift per pay period (every two weeks).
- Occasional travel radius of roughly 60 miles (60-90 minute total drive time) per day.
- Business casual attire no jeans scrubs allowed but not required (no specific color).
Scheduling:
- 8-hour day shifts.
- Shifts are determined and assigned by the facility.
- No weekend requirement.
- No on-call requirement.
- No block scheduling.
