Job Summary:
The Registered Nurse provides skilled nursing care case management and hospice care to geriatric patients facing end-of-life care in Snohomish County. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team working with an average of 4-6 patients per day.
Location:Everett Washington United States
Responsibilities:
- Provide skilled nursing care and case management to patients.
- Conduct patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Complete documentation by 9 AM the following morning.
- Travel within a 60-mile radius from the hub/office to visit patients.
Required Skills & Certifications:
- Active Registered Nurse license.
- BLS certification.
- Minimum of 1 year of recent experience.
- Experience with HCHB EMR.
- Clinical Assessment Medication Administration and Treatment Planning skills
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Hospice care experience.
Special Considerations:
- Weekend coverage may be needed to be discussed during the interview.
- No jeans allowed in dress code; scrubs of any color are acceptable.
- Must use provided iPad for documentation; personal cell phone is not to be used for work purposes.
Scheduling:
- Guaranteed 40 hours per week.
- Open schedule options: Monday-Friday Tuesday-Saturday or Wednesday-Sunday.
- Shifts to be determined.
