Hospice Registered Nurse(RN)
PRN
Coverage Area: Northwest & Southwest Houston
Responsibilities:
- Oversee nursing care team.
- Communicate and collaborate with physicians therapists and other healthcare professionals to ensure cohesive and comprehensive patient care.
- Plan organize and direct home care services.
- Conduct assessments of patient care needs and develop care plans.
- Educate patients and their families on managing their health conditions medication regimens and any necessary lifestyle changes.
- Prepare timely complete and accurate clinical notes.
- Night and Weekends are required per hospice needs of patients
Qualifications:
- Valid RN license in the State of Texas.
- Hospice experience (at least 1 year).
- Knowledge of home health regulations and general healthcare compliance.
- Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
- Excellent communication and people skills are essential.
- Ability to work independently and as part of a team.
