Home Health Registered Nurse Every Other Weekend
Job Type:Home Health RN- Every Other Weekend
Location:In Person Fairfield County CT
Pay Range:$65 $75 per hour
Schedule:Every Other Weekend
Position Overview:
We are seeking a compassionate and experiencedRegistered Nurseto provide comprehensive home health care services. The RN will manage care plans administer treatments and collaborate with multidisciplinary teams to support patients in their homes.
Key Responsibilities:
- Perform clinical assessments and provide nursing interventions including wound care medication administration and procedures
- Monitor and document changes in patient condition
- Execute and follow up on physician orders
- Collaborate with home health aides and other care providers
- Manage care coordination and ensure quality patient-centered care
Qualifications:
- Active Connecticut RN license
- Minimum 2 years of nursing experience (home health preferred)
- Reliable transportation for patient visits
- BLS Certification preferred
- Strong communication organizational and critical thinking skills
- Knowledge of OASIS
