Pay Rate: $28.00 $31.25/hr mileage reimbursement

Position Type: Special Projects 12-Month W2 Contract

Schedule: 9:00 AM 5:30 PM PST Mon Fri

Model: Field-Forward (multiple in-person visits weekly)

Star Nursing Services is hiring a Special Projects Lead Care Manager to support CalAIM Enhanced Care Management (ECM) and Community Supports operations with Alameda County. This is a one-year contract with renewal consideration based on performance and program continuation.

Due to SCFHPs member population we require candidates who are bilingual English (Mandarin or Cantonese or Spanish).

What Youll Do

• Lead Care Managers delivering Enhanced Care Management (ECM) and Community Supports

• Complete frequent in-person home visits assessments care plan updates and transitions of care

• Support escalations and collaboration with health plan partners

• Train new staff on CalAIM workflows county resources and documentation

• Review EMR documentation for accuracy and MCP alignment

• Support audit readiness findings and CAP implementation

• Partner with ECM clinical teams community agencies and housing providers

Important Field Expectations

• Multiple in-person visits every week

• Daily travel across Alameda County

• Remote work is limited

Required Qualifications

• Care/Case Management experience required

• Minimum 1 year in CalAIM ECM/CS case management public health or related fields

• Strong understanding of Medi-Cal Enhanced Care Management (ECM) Community Supports authorizations and managed care requirements

• Experience with appeals grievances audits CAP findings and corrective action plans

• Knowledge of Alameda County resources

• Strong communication and leadership skills

• Bilingual English (Mandarin OR Cantonese OR Spanish)

• 2 character references

Vaccine Requirements:

• Physical (within the last 30 Days)

• TB Test

• MMR Varicella Hep B or Titers- Vaccination

• Tdap/Flu (or decline)

• Covid Fully Vaccinated (or decline)

• Must be able to pass background/ urine drug screen

Transportation Requirements

• Unrestricted active California drivers license

• Valid auto insurance

• Current registration

• Reliable vehicle

Technology Requirements

• Intermediate skills in:

• Microsoft SharePoint

• Excel

• Word

• Teams

• Adobe (PDF editing forms signatures)

• Experience with EMR/EHR systems used for CalAIM documentation

Minimum PC Specifications

• Windows 10 or higher

• 8GB RAM minimum (16GB recommended)

• Intel i5 / AMD Ryzen 5 or better

• 256GB SSD or larger

• Chromebooks not allowed

• Screenshot of computer specs required for interview

Education

AA/BA or equivalent experience in case management housing social services or public health.
